This is an AGREEMENT made and entered into this day with Sound Extreme Entertainment. I agree to engage the services of Sound Extreme Entertainment.

I agree to pay Sound Extreme Entertainment the sum of $ on the same day as performance. A check in the amount above shall be payable to: Sound Extreme Entertainment.

This AGREEMENT shall commence on the date entered above.
This AGREEMENT is a legal and binding contract. Neither PERFORMER nor CUSTOMER may terminate this AGREEMENT without consent of the other PARTY or without a notice of termination in writing given to other PARTY at least six (6) weeks prior to the date of performance. If consent is not obtained from, or proper six week written notification is not given to the other PARTY, the PARTY terminating the AGREEMENT shall compensate the other PARTY for any losses incurred.
